John Bartlett-Powers

 

Colorado Health Connections is proud to welcome John Bartlett-Powers, MSW, to our education team. John is participating in and managing integrated care planning for the benefit of our patients.

John was born in Chicago, Illinois and has over 20 years of social work experience. He graduated from the University of Wisconsin in Madison, with a Masters of Social Work degree. He also commands a Bachelor of Arts degree and has attended the Metropolitan State College of Denver participating in the Elementary Education program.

John has been active as a VISTA volunteer in Boulder County, working with the developmentally disabled, Case Management with North Metro Community Services, Case Management with Mediplex Rehab, serving brain injured adults and has had social worker positions with Centura Senior Health Center and Porter Hospice. He has been involved with sensitively transitioning handicapped persons from institutions into the community and quality continuity of care for individuals served by senior clinics.

 

 

 

 

 

 

 

John has also served on the Jefferson County Adult Protection Review Committee, the Board of Senior Support Services and been recognized by Marquis Who's Who in America.

Colorado Health Connection's patients, referral sources, community and staff are now benefiting from the experience and skills that John is delivering with compassion and dedication. John is actively involved in providing in-servicing for Assisted Living communities, Independent Living communities and Skilled Nursing Facilities and their staffs, working along side of Colorado Health Connections to educate and coordinate innovative care.

John is married with three grown children and two Scottish Terriers. He enjoys travel and stamp collecting and is very active in the community.
