Our Values: The patient comes first!
In 2005, Robyn Edge, founder of Colorado Health Care, gathered like-minded health care professionals and opened Colorado Health Connections.
Today, the team has grown to include 29 individuals who bring a combined 400+ years of experience to the people they serve. They also bring a patient-centered approach to care.
At Colorado Health Connections, nurses, physical, occupational and speech therapists, certified nurse assistants and patient advocates form a team with each patient. By working together and putting the patient first, these committed health care professionals can provide the service or set of services that is most beneficial to each patient.
Team members take the leadership role in coordinating and communicating with other care providers who contribute to each patient’s healing and independence. This prevents fragmented care and decreases the likelihood of return visits to the hospital.
Colorado Health Connections provides as many services as needed to strengthen the link between health and care.
