  • GENERAL CARE

    Colorado Health Connections cares for a variety of individuals with multiple needs and challenges. We have an understanding and adaptable staff that works together to meet the specific needs of our patients and their caregivers. Our nurses provide extensive education about medications, disease processes, diet, infection control, and healing.

    Colorado Health Connections therapists provide an array of treatments tailored to the individualized goals of each patient. Our MSW/patient advocate provides counseling and community support to the patients and their families, and our CNAs offer safe and professional assistance with bathing and self-care needs. We are proud of the team of health care professionals that can help individuals find safety and wellness in their homes.

  • PHYSICAL THERAPY

    Our physical therapists consult with the patient and physician to help evaluate the patient’s current abilities. From there, our therapists help the patient develop and improve the physical skills needed to achieve and maintain maximum performance.

    The program may include:

    Home Exercise • Pain Relief • Ultrasound Therapy • Transfer Training • Equipment Procurement • Endurance Training • Gross Muscle Rehabilitation

  • OCCUPATIONAL THERAPY

    Occupational Therapists work with other team members to determine what the patient needs to live as safely and as independently as possible. The Occupational Therapist’s primary function is to evaluate the patient’s environment as well as their physical and mental abilities. The therapist then creates a plan that focuses on optimizing an individual’s independence in the home setting. This plan typically includes an exercise program, suggestions for environmental modifications, and simulating activities of daily living with the patient.

  • NURSING

    A registered nurse interviews and evaluates each patient to determine the specific needs of that patient. Drawing on decades of experience and specialized skills, our RNs coordinate communication among all the team members—the patient, physician, occupational and physical therapists, licensed practical nurses and others—to design and implement a preventive and rehabilitative plan of care that promotes autonomy and independence.

  • PATIENT ADVOCATE

    Our patient advocate, who is a medical social worker, assesses the home and socioeconomic environment of each patient to identify any roadblocks to healing. Our patient advocate knows the resources available within our community, including organizations that provide adaptive equipment, transportation, meals and other services. Working with the health care team, the patient advocate navigates the network of community resources and connects the patient with services needed to live well at home.

  • CERTIFIED NURSE ASSISTANT

    Certified nurse assistants provide the most personal of all health care services. They help patients with the intimate details of life that start and end our days: getting out of bed, bathing, getting dressed and going to sleep. Our CNAs have assisted hundreds of patients over the years and they fully understand and value each person’s privacy. Under the supervision of the Registered Nurse, our CNAs work with the entire team to help each patient enjoy life.
