The team at Colorado Health Connections is excited and honored to be invited into your home to help with your health care. And we’d like you to know what to expect on your road to recovery.
When we receive a call from your physician, a hospital representative or family member, we begin our process of learning as much as we can about you and your specific needs.
We will call you within 24 hours to let you know we’re on your team. Within the first two days, we will send a registered nurse or therapist to meet with you to discuss your goals and how we can help you meet them.
Once the staff member has met with you and created a plan of care, they will contact the other team members needed to assist your recovery. Each team member will contact you to introduce themselves and schedule time with you. We also communicate with your physician and other health care providers to assure that your care is comprehensive and well integrated.
