
Colorado Health Connections has extensive experience working with individuals in need of cardiac care. Our nursing staff is able to provide QD/BID (daily and twice-a-day) treatments consisting of blood pressure checks, weight management, post-surgery wound management, protime venipuncture, and patient/caregiver education for medication and disease process. Our therapists provide cardiac rehabilitation exercise programs and educate patients on methods to safely maximize cardiac output. Therapists discuss energy conservation and also recommend any necessary durable medical equipment. Our CNAs provide assistance with bathing needs until the individual is able to safely bathe themselves. Finally, our MSW/patient advocate provides counseling and information on community networks and all needed resources for success.
Colorado Health Connections is equipped with staff who provide exceptional cardiopulmonary care. Our nurses are available for daily visits, protime blood tests, medication education, and patient and caregiver education about dietary needs. Therapists provide patients with functional endurance training as well as strength training in order to maximize their physical capacity. We work with patients to create a personalized treatment plan designed to meet their unique needs.
Colorado Health Connections is proud to employ WCC nurses. We provide QD/BID (daily and twice daily) nursing treatments if requested by the patient’s doctor. We complete co-morbidity assessments and education, disease process education, medicine education, and dietary education to our patients and their caregivers. Our nurses and therapists follow specific wound care procedures necessary for a variety of healing processes. We also specialize in therapies that optimize blood flow and endurance through the healing process.
Colorado Health Connections works in conjunction with the patient’s orthopedic surgeon to follow their specific, personalized rehabilitation protocol. We have a team of educated therapists, nurses, and patient advocates who provide assistance with muscle rehabilitation, scar management and infection control, optimal independence and daily living safety, and medication education. We provide patients with information about the medical equipment they will need as they rehabilitate in their homes. We also follow through with the rehabilitation process by referring patients to any necessary out-patient and in-home recovery assistance services.
Following surgery, it is important to know that you have great support at home. Colorado Health Connections provides daily nursing when needed, venipuncture follow-up, post-surgery wound care, pain management, and infection monitoring/control for our post-surgery population. We monitor patients’ dietary needs and identify their individual safety issues. We provide patients with a post-surgery exercise protocol, and work in conjunction with their primary care physicians and surgeons to provide optimal communication and follow-up. We are the link to a successful and safe return to the home following surgery.
Colorado Health Connections strives to provide the best care for our patient populations with diabetes, adrenal gland disorders, thyroid disorders, osteoporosis, and multiple other endocrine disorders. We provide extensive diabetes management and education, cholesterol management, co-morbidity assessments, and blood sugar education. Our nurses specialize in wound care therapies that maximize healing. We educate patients on their personal dietary needs as well as disease processes. We provide patients with information about medical equipment, monitors, and exercise programs to improve their day-to-day wellness. We also provide extensive therapy for individuals with prosthetics and amputations.
Colorado Health Connections works with a variety of neurological diagnoses, including but not limited to MS, Parkinson’s, ALS, TBIs, and CVA. We provide patients and their families and caregivers with education on medication and the recovery process. We also complete cognitive assessments, give recommendations for durable medical equipment, provide community resource information, as well as work with therapies that maximize strength and wellness. Ultimately, we create a roadmap to help guide patients, along with their caregivers, to find their optimal function.
