We provide nursing care for short-term or ongoing care needs, ranging from basic wound care after surgery to complex, high-tech care. Our rehabilitation and therapeutic services include physical, occupational and speech therapy. We also provide assistive care services that help clients with self-care needs so they can continue living independently at home.
- Your doctor decides that you need medical care in your home, and makes a plan for your care at home.
- You need at least one of the following: intermittent (and not full-time) skilled nursing care, or physical therapy or speech language pathology services.
- You are homebound. (This means that you are normally unable to leave home. Being homebound means that leaving home is a major effort.)
- When you leave home, it must be infrequent, for a short time or to receive medical care.
In addition to having a physician’s referral, you need to have a safe home environment to provide care in, and be a willing patient. Patients must also be homebound, meaning that it requires considerable and taxing effort to leave home.

Colorado Health Connections coordinates and accepts reimbursement from the largest group of health insurance payors in the country. This includes Medicare, Medicaid, workers compensation, other government programs, private health insurance, auto insurance and HMOs. We also accept private pay. Our goal is to give you added peace of mind by simplifying the process. We have reimbursement specialists who can handle all the paperwork for you, contact payors to determine levels of home health coverage, and then bill the appropriate plan directly.
